Students can submit the Brock University Student Absence Self-Declaration Form twice per course per term. The form acts in place of a doctor’s note for short term illnesses lasting 72 hours or less. While not required, instructors may find it helpful to facilitate student use of this form through Brightspace, where submissions can be tracked and recorded.

Tracking Student Absence Self-Declaration Form Submissions

CPI recommends making use of the Assignments tool to track submissions of the Student Absence Self-Declaration form. Instructors can create an assignment with the form file attached, and set the assignment to only allow for one file and one submission. This will make it so students can only submit the form once across the duration of the course, because students are permitted to submit the form twice per course per term, and Brightspace doesn’t have a limit of 2 submissions, instructors will have to copy the assignment and create another for the second submission.

Download an Assignment Package

CPI has created a .zip file containing a pre-made assignment section for the Student Absence Self-Declaration forms. Instructors need only download the zip file, then import it into their site as a Brightspace package using the Import/Export/Copy Components tool.

The file is here:

CPI provides import steps in our Import/Export/Copy Components article.
