In the case of students becoming ill, they can be granted the use of The Brock University Medical Self-Declaration Form. The form acts in place of a doctor’s note for short term illnesses lasting 72 hours or less. While not required, instructors may find it helpful to facilitate student use of this form through Brightspace, where submissions can be tracked and recorded.

The Medical Self-Declaration Form form can be downloaded or linked to online.

Tracking Medical Self-Declaration Form Submissions

CPI recommends making use of the Assignments tool to track submissions of the Medical Self-Declaration form. Instructors can create an assignment with the form file attached, and set the assignment to only allow for one file and one submission. This will make it so students can only submit the form once across the duration of the course.

Download an Assignment Package

CPI has created a .zip file containing a pre-made assignment for the Medical Self-Declaration form. Instructors need only download the zip file, then import it into their site as a Brightspace package using the Import/Export/Copy Components tool.

Instructions for Creating an Assignment

For instructors who would prefer to create an assignment themselves, the basic steps are listed below.

  1. Create an Assignment.

  2. Insert instructions for the students to follow regarding filling out the form.

  3. Upload the form as a file attachment for students to download ( ).

  4. In the Submission and Completion sub-menu, set Files allowed per submission to One File, and Submissions to Only one submission allowed.

  5. Once done editing, click Save and Close.
