Include Instructors can create up to five custom likert-style or open-response questions in your a Student Course Experience Survey. Personalized questions do not replace the default survey components and will appear at following the standard Brock-wide questions and any provided by a Department or Faculty.
The quickest way to view all existing questions is to Preview the Survey.
Use custom questions to gather feedback about elements of your a course’s design that are emergent or that are not addressed by the questions already present in the survey form.
Personalized questions will appear at the end of the survey and will not replace questions that are already present.

Add personalized questions
Personalized questions must be added to each survey as it is listed, even if they are multiple sections of the same course.
Find the Question Personalization option associated with the course
to be modified and click on the three dot button on the far-right. This option will display as Expired once the survey for the course section has become available to students
If your course includes multiple sections, take this step for each section to which you’d like to add personalized questions.

Up to five questions in total can be added to either of the two options:
Create custom-worded rating questions
for Likert-style rating questions.
Create custom-worded open-ended questions for written response questions

Click Customize
Enter text
Enter the text of your question within the space that’s provided and ensure that the Select option has been enabled. Additional questions can be added to this section after clicking their associated Edit buttons.
Submit questions
Ensure that the Submit button is used after you’ve completed entering your personalized questions.

Both types only allow for custom question text. The rating options cannot be changed.
Click Expand all to proceed to the editing options in either section.
Click Customize to add custom question text.
Enter the question text within the Question field [1]. Click the close icon [2].
Ensure that the Included status is selected for any new questions.
Click Submit to save the personalized questions as part of the survey.
